Medical Records Collection, Retention, and Access in New Hampshire

Health care professionals and facilities must maintain accurate records for each patient and keep them for a specific number of years depending on the type of health care professional or facility.1 Pharmacies and other health care facilities must comply with laws specifically pertaining to medical records and prescriptions and controlled substances.2 Health care providers must also maintain clinical records to support claims submitted for Medicaid reimbursement.3 The information contained in a patient’s medical record is confidential but the patient may access his or her medical information and request copies for a reasonable fee.4
