Each licensed ambulatory surgical treatment center (ASTC) must report the following data elements for each discharge regardless of payer:
1. Patient identifying information;
2. Patient contact information;
3. Patient demographic information, including age and sex;
4. Patient’s insurance or third party payer information;
5. Patient’s diagnosis or treatment information;
The ASTC may choose to report the patient’s race and ethnicity.
