An individual has the right to review his or her own personal records maintained by the State Department of Public Health if the individual submits the request in writing.  The department has established procedures for reviewing requests from individuals who seek to review their records.  If a person is denied access, he or she may request an administrative hearing.  The department must keep track of all requests it receives to review personal records.
