An insurance company is required to maintain a clinical patient record. For treatment provided on an inpatient basis, the responsibility is on the health facility to maintain the patient record and provide it to the insurance company if needed. In order to accurately and thoroughly maintain medical records, there must be enough staff, facilities and equipment.
