Any patient undergoing surgery must have a medical record that is signed by the attending physician. The hospital-operated facility must maintain retention policies for the medical records. The medical records must include patient identification, medical history and examination information, and any laboratory findings, among other requirements. The Department must have access to medical records at any time. Medical records must remain confidential, but exceptions exist for information required by these rules or by law and information authorized for release by the patient.
