Mich. Admin. Code r. 325.165 - Amendment, replacement, and removal of records

A provider may amend information in the registry so long as they provide the correct authorization code and include the replacement information on Department-approved documents. The subject of the records may also amend the registry if they apply to the Department. If a request is made the make immunization records inaccessible, the Department must do so. The Department must also maintain records of all of the changes made to the records, including the date of the change, the item changed, and the individual who requested the change.
