Patients receiving mental health and substance use treatments must be provided with a treatment plan. The attending physician must assess the patient’s social and psychological needs when creating a treatment plan. The treatment plan must be recorded in the patient’s medical records and any changes to the plan must be reflected in the patient’s medical records. Treatment plans must also be reviewed at least once every 90 days.
