The Rhode Island General Assembly recognizes that continued use of the patient centered medical home (PCMH) model is “critical to the future structure of the Rhode Island primary care delivery system.” The General Assembly defines the PCMH as “a health care setting that facilitates partnerships between individual patients, and their personal physicians, physician assistants and advanced practice nurses, and when appropriate, the patient's family.” Characteristics of the PCHM include delivering high quality care, encouraging patient participation, and utilizing health information technology.
