The director must create a program for providing individuals with long-term care assessments at no cost. The assessments must provide individuals with information regarding community support services, residential placement alternatives, and care recommendations. Assessments are not binding; rather individuals may choose from any available options.
The department must establish a “care management services program” to provide “guidance, supervision and monitoring of” home and community based care and to periodically reassess the adequacy of an individual’s care plan.
