All chiropractic physicians must create and maintain a chiropractic record for every patient receiving services.  These records are separate and distinct from those records of chiropractic services created by or at the request of a hospital that are under the control of the hospital.  All chiropractic records must be retained for at least ten years from the date of the physician’s last contact with the patient.
