Records – Ohio Rev. Code Ann. § 5119.06

The department of mental health must keep in its office, accessible only to its employees, a record with the following information for every patient in the institutions governed by the department:

  • Name;
  • Residence;
  • Sex;
  • Age;
  • Nativity;
  • Occupation;
  • Condition;
  • Date of entrance or commitment;
  • Date, cause and terms of discharge;
  • Condition of such person at the time of leaving the institution;
  • All transfers from one institution to another; 
  • Date and cause of death, if such person dies while in the care or custody of the department.

In the case of an accident or injury or peculiar death of a patient, the managing officer of an institution will make a special report to the department within 24hours, giving the circumstances as fully as possible.
