Or. Rev. Stat. § 444.320 - Physician report of childhood diabetes

A physician who has diagnosed a child under 18 years old with Type I or Type II diabetes must report to the Oregon Health Authority the following information:

  1. The name and address of the child;
  2. Age of the child;
  3. Date of birth of the child;
  4. Type of diabetes; and
  5. Date of diagnosis or first treatment.
