The Oregon Health Authority will annually conduct a survey, to be completed by June 15, of all public and public charter schools, to collect data about diabetes occurring in students. Each school surveyed will report to the authority for each student enrolled at the school who has Type 1 or Type 2 diabetes the following information:
- The name and address of the student;
- The gender of the student;
- The date of birth of the student;
- The type of diabetes diagnosed; and
- The date of diagnosis
