Each induced termination of pregnancy or abortion that occurs within the State of Oregon must be reported to the Center for Health Statistics within 30 days by the head of the facility where the abortion was performed or by the attending physician. Additional follow up visits for complications should also be noted in the report. Any report submitted must not identify the patient in any way.
