The Oregon Health Authority must specify which diseases must be reported, identify the entities or people who must report, prescribe the reporting method, and come up with methods of investigating the diseases.  All reporting must be to the local public health administrator, who then reports to the Oregon Health Authority.  Either the local public health administrator or the Oregon Health Authority must investigate the reportable disease or outbreak by interviewing individuals or by requiring providers to give access to records.  Information requested by the Health Authority may include individually identifiable data.
