Mich. Comp. Laws Ann. § 333.5611 - Report of occupational disease or health condition aggravated by workplace exposures; time; contents; forms and instructions

All physicians, hospitals, and clinics aware of or suspecting a case of an individual’s occupational disease must report the case to the department within 10 days of discovery. The report must indicate the individual’s name and address, the employer’s name and address, and the nature of the disease, among other requirements.
