Requires the Department of Health and Welfare to present a Medicaid Managed Care Plan to Idaho’s legislate that focuses on high-cost populations such as dual eligibles and high-risk pregnancies. Specifies the minimum elements that the plan must address (e.g., use of primary care medical homes to improve care coordination, development of medical homes for populations with chronic disease, reduction of duplicative practices that increase costs, etc.).
