Idaho Code Ann. § 39-4804, Notification to Parent or Guardian

Specifies the information that a health care provider must disclose to a child’s parent or guardian prior to administering an immunization (e.g., participation in the immunization registry is optional, immunizations may be refused for religious or other reasons, etc.). Specifies the information that providers must disclose to parents or guardians when collecting information for the immunization registry (e.g., their right to file a statement regarding their exemption from the registry, their right to remove information from the registry, etc.). Prohibits the fact that a parent or guardian refused to have their child immunized, filed a statement regarding their exemption from the registry, or requested the removal of their child’s information from the registry from being used against the parent or guardian in a legal proceeding.
