Requires health carriers to provide covered persons with notice of their external review procedures. Permits such notice to accompany the policy, outline of coverage, or other document the carrier uses to establish evidence of a person’s coverage. Requires health carriers to provide covered persons with notice of adverse determinations and to accompany this notice with information about the person’s right to seek an external review and a form to obtain that person’s authorization to disclose protected health information. Provides the language that these notices must include in Appendices A and B, but permits the Insurance Director to authorize notices that use substantially similar language to the language in the appendices. Requires the authorization to disclose health information form to comply with 45 C.F.R. 164.508.
