Idaho Admin. Code r. 16.05.01.125, Access to an Individual’s Own Record

Permits individuals aged 14 or older—or their authorized representative— to access a copy of the individual’s records by submitting a written request to the Department of Health and Welfare. Requires this written request to identify the person whose information is sought and describe the information sought. Specifies the circumstances in which individual may not access their information (e.g., information about adoption, information that a health care provider believes would endanger the individual or another person, etc.).
