Idaho Admin. Code r. 16.03.10.704, Adult DD Waiver Services: Procedural Requirements

Requires the development of a plan of service for participants who receive adult DD waiver services. Requires providers of such waiver services to maintain records and specifies the content of such records (e.g., time and date of service visits, changes in the participant’s condition, etc.). Requires services providers to notify the plan creator or coordinator if the participant’s condition changes significantly and to document that they provided such notice in the service record. Requires the Department to obtain a participant’s records when the participant changes providers and safeguard them until a new provider is selected. Requires the Department to retain participant records after the participant leaves the waiver program. Requires provider agencies to retain participant records for at least five years after the last date of service.
