Establishes standards for transition management services provided to participants who are transferring from a facility to a community setting. Tasks transition managers with serving as a liaison between the participant, their old facility, the Department of Health and Welfare, and other persons designated by the participant as the participant transitions into their new community setting. Defines the specific services that transition managers may provide as including the performance of a comprehensive assessment of the participant’s health, social, and housing needs, helping the participant apply for public assistance, and securing needed medical supplies and equipment. Limits each participant to 72 hours of transition management services.
