Idaho Admin. Code r. 16.03.10.328, Aged and Disabled Waiver Services: Procedural Requirements

Requires the Department of Health and Welfare—or their contractor— to develop an individual service plan for participants who are receiving aged and disabled waiver services. Requires the Department to administer the Uniform Assessment Instrument (UAI) to these participants. Clarifies that services not included in the service plan and those provided in excess of the services in the service plan are not eligible for Medicaid reimbursement. Requires all waiver services to receive prior authorization from the Department. Establishes standards and criteria for the individual service plan (e.g., the waiver participant must help in creating the plan, the plan must be revised at least annually, etc.). Requires providers to maintain records for each waiver participant and specifies the information that must be documented in these records (e.g., date and time of visit, changes in the participant’s condition, etc.). Requires providers to maintain records for at least five years after the last date of service.
