Identifies the elements that home and community based person-center service plans must include (e.g., necessary services, service delivery preferences, participant goals, etc.). Requires the participant or their decision-maker to sign the plan as well as any provider tasked with implementing the plan. Requires each of these parties to obtain a copy of the plan.
