Requires a child’s primary RN to submit a plan of care to the Department of Health and Welfare annually for reauthorization. Requires physicians to provide the Department with EPSDT screening information, order the services that the PDN will provide, review and sign the plan of care, and make a determination as to whether PDN services and community resources are enough to assure the child’s health and safety. Requires physicians to notify the Department and the family if they believe the child needs to be admitted to a medical facility.
