Identifies the dental services for children covered by Colorado’s medical assistance program. Includes two oral evaluations per year for children aged 0-2 and 3-4 and two fluoride varnish or fluoride gel treatments per year for children aged 0-4. Permits an additional two evaluations or fluoride treatments for children at high risk of caries. Requires providers to perform a risk assessment for caries and document the assessment in the child’s medical record.
