Reports of Physicians and Health Officers Regarding STDs

Every physician who diagnoses, treats, or prescribes for a case of a STD and every superintendent or manager of a clinic, hospital, laboratory, or penal institution in which there is a case of a STD shall report the case immediately to whomever the commissioner designates to receive that information. The Department shall provide forms that require the name, address, age, sex, race, stage of disease, and other information in order to provide for the location, treatment, and control of infectious cases. Only STDS that are designated by Department regulations are required to be reported.
