School reporting of body mass index and weight status category – Ohio Admin. Code 3701-65-01

The board of education of a city, exempted village, local school district and governing authority of a chartered nonpublic school must annually report to the department of health the following information for children enrolled in kindergarten, third grade, fifth grade, and ninth grade:

  • Each student’s body mass index and weight status category;
  • The grade level of each student; and
  • The sex of each student.
