Revocation - R.I. Gen. Laws Ann. § 23-4.10-3

A person may revoke their durable power of attorney “at any time and in any manner by which [they are] able to communicate an intent to revoke…” However, the person revoking their power or attorney or a witness to the revocation must communicate the revocation to an attending physician, health care provider, or emergency services personnel in order for the revocation to be valid as to that physician, provider, or emergency personnel.

An attending physician or health care provider must document the revocation of a power of attorney in the person’s medical record.
