Every institution governed by the department of rehabilitation must compile, maintain and keep separate a medical record for every inmate in the institution. Upon the request of both the inmate and his designated attorney or physician, the department will make his medical record available the attorney or physician for inspection or copying.  The department may charge a reasonable fee for the copying of medical records.  If an inmate is under potential serious medical harm, the physician shall indicate on the medical records.
