Plan for a system of preventive health services for children under the HUSKY Plan, Part A and Part B - Conn. Gen. Stat. § 17b-306

The Commissioner of Social Services must design a plan for implementing a “system of preventive health services for children under the HUSKY Plan, Part A and Part B” as a means of improving health outcomes and reducing racial and ethnic health disparities. The plan must: (1) create “a coordinated system for preventive health services” that includes, among others, care coordination and chronic disease management; (2) mandate that the Department of Social Services track the utilization of services by HUSKY Plan beneficiaries and beneficiary health outcomes; (3) establish financial incentives for participating providers (e.g. pay for performance, case management fees).
