Pharmaceutical Case Management Services
 
Permits physicians and pharmacists to provide pharmaceutical case management services to Medicaid recipients if they are enrolled in Iowa’s Medicaid program, undergo appropriate training to provide pharmaceutical case management services, maintain case management records, and permit the Department of Human Services access to these records. Permits Medicaid recipients to elect pharmaceutical case management services if they regularly take at least four nontopical medications and have one eligible disease (e.g., diabetes, heart disease, etc.). Identifies the services that a pharmaceutical case management team must provide as an initial assessment, new problem assessments, problem follow-up assessments, and preventive follow-up assessments.
