Establishes qualifications for health home services providers’ participation in Iowa’s Medicaid program. These qualifications include the use of HIT to maintain medical records, collaborating with case managers, and providing integrated services. Requires participating providers to electronically report quality measures to the Iowa Medicaid enterprise.
