Patient Authorization for Disclosure
 
Permits patients to authorize the disclosure of their health care information and requires hospitals to honor patient requests to disclose information subject to 35-2-612 (specifies the circumstances in which hospitals may deny access to health care information). Allows hospitals to charge a reasonable fee for providing the information so long as the amount does not exceed the costs incurred. Defines a valid disclosure authorization as one that is written, dated, signed by the patient and identifies the type of information to be disclosed and the persons who will receive the information. Limits the duration of an authorization form to 12 months after the date of signing or 48 months for authorizations to disclose information to third party payors.
