Or. Admin. R. 410-121-4010 - Reporting Requirements

Within one week of dispensing a controlled substance to a Division of Medical Assistance Program fee-for-service client, pharmacies must electronically report to the Division the following information:

  • Patient's full name, address, and date of birth;
  • Pharmacy Drug Enforcement Administration Registration Number;
  • Prescriber name and Drug Enforcement Administration Registration Number;
  • Identification of the controlled substance using a national drug code number;
  • Date the prescription was written;
  • Date the drug was dispensed; and
  • Number of metric units dispensed.
