Information contained in the Physician Orders for Life-Sustaining Treatment (POLST) Registry may only be released to authorized persons, facilities and researchers, and for authorized purposes. All information collected or developed by the registry that identifies or could be used to identify a patient, health care provider or facility is confidential. Only the minimum amount of information needed to accomplish the intended, authorized purpose shall be released.
