All information collected via the Oregon Death with Dignity Act, including the identity of patients, is not a public record and may not be made available for inspection by the public. Both the information collected and the annual statistical report will be considered a special morbidity and mortality study; summary information released in statistical reports may be aggregated to prevent identification of individuals, physicians and health care facilities.
