Office of Health Equity Rules for the Health Disparities Grant Program
Establishes standards for Health Disparities Grant Program applications and criteria for selecting applicants to receive funding. Requires grantees to create a written evaluation plan that identifies items such as how the grantees will accomplish their goals, how they will measure success, the target population, the provided services, and their plan for addressing cultural competence. Requires grantees to provide the Department of Public Health and Environment with quarterly reports and a final evaluation report.
