The State Department of Public Health must maintain the confidentiality of the reports submitted to the Cancer registry and ensure that the reports are de-identified.  The Department may also release information received from the Cancer registry to authorized researchers reviewed by the scientific advisory panel. 

If reports have individually identifiable data in them they may be released to the specific individual upon a signed, written request or, if the individual is a minor, to his or her parent upon signed, written request and birth certificate of the particular child or other proof.  If the individual identified in the record has a court appointed guardian or an attorney, a signed, written request, official documentation appointing the guardian or attorney, and proper identification of the guardian or attorney.
