Requires nursing facilities to conduct an admissions assessment for new residents and make this assessment part of the resident’s record. Requires facilities to create a discharge plan and a mental health plan of care for residents and for these plans to be made part of the resident’s record. Identifies the elements that the mental health plan of care must address (e.g., services provided, medical directives, service providers, etc.) and the persons that must be involved in creating the plan (e.g., interdisciplinary team, the resident, etc.).
