A report must be made any time an event occurs that may result in the injury of a resident, staff or visitor.  This can include medication reactions and errors involving a resident.  Any resident who experiences an injury or accident must be examined by a physician unless a Registered Professional Nurse determines otherwise.  The extent of injury and treatment must be recorded on the resident’s record, with notification given to the nearest relative or guardian of the resident.  All incident and accident reports must be initialed within 24 hours of occurrence, and must be kept on the premises of each facility to be reviewed at each meeting of the Quality Assurance Committee.
