Specifies the information that must be included in communicable disease reports (e.g., the name and address of the person reporting, the diagnosed disease or condition, name and contact information of the person with the disease, etc.). Permits persons to report communicable diseases to the Department of Health and Welfare or their Health District by phone, fax, mail, or an approved electronic reporting system. Permits after hours reporting through the State EMS Communications Center.
