HIV - related conditions -- consent, testing, and reporting -- penalty. 
Requires health care providers to obtain a person’s consent prior to administering a voluntary HIV test. Permits the use of a general consent form that covers all medical procedures, but requires a specific HIV test consent if the general consent is no longer valid. Requires providers to report positive HIV tests, diagnoses of AIDS or AIDS related conditions, and deaths of persons with HIV to the Department of Public Health within 7 days of the reportable event. Requires blood banks and clinical laboratories to report positive HIV test results to the Department of Public Health. Specifies the information that must be reported (e.g., name, address, race and ethnicity, etc.).
