Head and Spinal Cord Injury Reporting Requirements - Mo. Code Regs. Ann. tit. 19, § 30-41.010

All physicians and hospitals must report to the Department of Health registry any of the following:

  1. A head or spinal cord injury patient who is dead or dies upon arrival to an emergency department;
  2. Any head or spinal cord injury patient who is transferred to another hospital from the emergency department;
  3. Any inpatient admitted to the hospital to treat a head or spinal cord injury. 

The registry form must include the following information:

  •         Hospital name and medical record number
  •         Patient name
  •         Social security number
  •         Date of birth
  •         Sex and race
  •         Previous treatment for the same injury
  •         If a minor (under 18), the above information for the parent or guardian is required
  •         Date and time on injury
  •         Cause of injury
  •         Scene of injury
  •         Hospital arrival information
  •         Hospital stay information
