“Fraudulently obtaining goods, services, etc., from a health care provider under the crimes law”

An individual who fraudulently obtains goods, products, or services from a health care provider is guilty of a misdemeanor of the 2nd degree. If he or she gave a health care provider a false name or address or invalid health insurance information, then these acts prima facie evidence of his or her intent to defraud the health care provider.
