“Disclosure of financial interest by production under the regulations and occupations law”
A health care provider must not refer a patient to a health care facility in which the provider is an investor unless the provider furnishes the patient with a written disclosure form prior to the referral. A violation of this statute is a misdemeanor of the first degree.
