“Communicable disease and AIDS prevention and control under the public health law”

The Department of Health must establish an immunization registry that contains information on whether children are immunized. The parent or guardian of a child may refuse to have their child included in the immunization registry by signing a form obtained from the department or from a health care provider that provides the immunization. Health care providers may access the immunization registry to view immunization records and update a child’s immunization history.
