“Authorizations under the Department of Financial Services regulations”

A valid authorization used by an insurer must be in written or electronic form and contain the enrollee’s identity, a description of the health information to be disclosed, a description of the recipients of the information, the enrollee’s signature, and notice of the length of time for which the authorization is valid. An enrollee may revoke the authorization at any time.
