Maine defines a sentinel event as the following:

  1. An unanticipated patient death or transfer to another health care facility unrelated to the patient’s underlying condition;
  2. A major permanent loss of function unrelated to the patient’s natural course of his or her underlying condition;
  3. An unanticipated perinatal death or permanent loss of function in an infant unrelated to the natural course of the infant or mother’s illness or underlying condition;
  4. Other serious and preventable events identified by a nationally recognized quality forum.
