Clinical Records
 
Specifies the records that skilled nursing facilities must maintain (e.g., physician orders, lab reports, treatment records, etc.). Requires facilities to maintain these records for at least six years and provide records to another facility when transferring a client. Specifies the information that must be documented in patient records when making such transfer (e.g., transfer form, nursing care plan, physician orders, etc.). Classifies client records as confidential, limits disclosure to authorized persons, and requires clients or their representative to sign an “Authorization for Release of Medical Information” form upon their admission. Requires facilities to employ a records administrator or assign responsibility for record keeping to an existing employee.
